Our Advance Disease Management is unique because its activity is triggered frequently before a claim, or even a diagnosis is made. Activity for most case management firms is triggered by a diagnosis or large claim amount. Engaging case management early at the point of lab tests gets faster and better results. Within hours of receiving lab and personal results from CareHere's Health Risk Assessment, ongoing lab tests and physician referrals, case management personnel contact the patient to start advance case/disease management.

CareHere health coaches work with employee/patients to:

  • Customized educational materials based on clinical values, learning styles, educational levels, and personal stage of change.
  • Local and personal experts are made available to encourage and monitor behavioral changes.

Our Wellness team will establish relationships within your community with health related professionals including dietitians, fitness experts, and mental health providers. These professionals coordinate with health coaches to ensure continuity of care.

Wellness Initiatives result from analyzing key health concerns affecting the employee/patient population. Examples include obesity/physical inactivity and smoking cessation. We coordinate the "field" activity and personnel with onsite clinic medical staff to achieve optimal results.
